A form of healthcare delivery in which the care is delivered by a group of physicians and other providers for a fixed monthly rate.
A group model HMO or multispecialty association of physicians and other health professionals that accepts a fixed payment from a managed care plan to provide a wide range of preventive, diagnostic, and treatment services to a defined population; the fixed amount is determined in advance for each year.
