Individualized transition plan developed by an educational team to establish specific transition goals to support a student's post-school goals
Individualized transition planning. a statement of needed transition services is required beginning at age 14. Transition services are defined as a coordinated set of activities for a student, designed within an outcome-oriented process, which promotes movement from school to postschool activities including postsecondary education, vocational training, integrated employment, continuing and adult education, adult services, independent living, and/or community participation.
Individual Treatment Plan
Individualized Transition Plan. An articulated, interagency educational plan designed to facilitate a student's move from school to employment and a quality adult life. The IEP/ITP addresses critical aspects of a student's transition, including employment goals, residential placement, guardianship, transportation, independent living, and income support. An ITP must be done in conjunction with an IEP.
Individualized Transition Plan. A plan developed by the IEP team to help accomplish the student's goals for the transition from high school into adulthood.
A written document developed by the Case Conference Committee, which contains the desired post-school outcomes in community living, employment or post-secondary education and training and the transition activities to occur for the remainder of a student's educational program.

Individual Transition Plan
Individualized Transition Plan. The Individualized Transition Plan is the part of a person's Individualized Education Plan (IEP) that identifies the long range goals of the person in respect to life after school. Transition services are a coordinated set of activities that are designed to help a student with disabilities move from school to life after school. The person's life after school may include: post secondary education, vocational training, integrated employment, continuing and adult education, independent living, participation in the community, and other activities. The ITP indicates how the individual will be supported or helped to participate in his/her preferred activities and achieve his/her goals.
